Insurance Benefits
It is important to contact your health insurance carrier, as well as our office, to confirm participation with your insurance plan. It is our policy to verify your insurance coverage before your appointment. At that time it will be confirmed if we participate with your insurance carrier.
At the time of your appointment, you will be asked to pay your co-pay, deductible, or co-insurance portion. You will need to bring your photo ID and insurance card to your appointment.
If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit. Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.

Out of Network
Often there is a network symbol on the back of your insurance card that enables you to receive health care services out-of-network. We will work with you and your out-of-network benefits or provide a fee schedule for you to see our physicians on an out-of-network basis. If we are out-of-network with your insurance plan, payment in full is expected at the time of service.
Self-Pay
We also provide medical care to self-paying patients. We will be happy to provide a fee schedule for you to see our physicians on a self-pay basis.
